Hormones can be implanted under the skin using a method developed over 60 years ago. After injecting a local anesthetic, a small incision is made in the skin of the lateral buttock. Tiny pellets made of bioidentical forms of estrogen and testosterone are placed through the incision into the subcutaneous fat. The pellets gradually release hormones over the next 4-6 months, relieving menopausal symptoms, improving energy and libido, and increasing bone density.
Pellets are superior to oral, transdermal and sublingual hormone formulations. They are effective for several months and do not require daily dosing. Additionally, they provide a constant blood level of hormones, avoiding the daily fluctuations that occur with all other methods of administration. Women who continue to have symptoms despite changes in hormone doses and the mode of administration are likely to respond more favorably to hormone pellets. Studies have shown that pellets are the most effective way of increasing bone density, four times stronger than hormones given by other routes. Women with severe osteoporosis should use pellets immediately and for the long term.
Testosterone is usually provided along with estradiol because it offers additional benefits of enhanced libido, improved energy and muscle strength, and headache relief. Men can also be treated with hormone pellets to treat testosterone deficiency. The dose is several times higher than in women.
Women who have not had a hysterectomy will also require progesterone supplementation to prevent endometrial proliferation. Progesterone may be taken as an oral capsule, at night to help with sleep.
The pellets begin to alleviate menopausal symptoms within several days. Blood levels are often measured after four weeks to verify effectiveness. The level of FSH, a pituitary hormone that increases when estradiol is low, should decline by 50% with proper hormone dosing.
