Abstract

Our musculoskeletal system is a complex structure which provides shape and helps in mobility from one place to another as well within the bodily structures. Ligaments, muscles, tendons, bones altogether move over each other to facilitate movements. Now all these structures ligaments, tendons, skin, muscles slide over the bones while joint movements. Slippery and tiny pouches filled with fluid known as bursae helps in this sliding motion by rendering a fine cushion and minimising the friction between the surface of these structures. Bursitis Trochanterica usually refers to the inflammation of the bursae in the hip joint. In the following text we shall discuss Bursitis trochanterica in a little detail and its Ayurvedic concept as well. We shall also have a look at its management in the contemporary system of medicine as well as Ayurvedic management.

Introduction

Bursitis is the condition when the outer lining of the bursae (also called synovial membrane) that facilitates joint movements get inflamed. Inflammation of the trochanteric bursa ( the greater trochanter) is called Bursitis trochanterica. There are a number of factors that can lead to Bursitis trochanterica. The condition usually occurs in women. Trauma and stress injury remains on the top of the list that causes Bursitis trochanterica. In Ayurveda, the condition of Bursitis trochanterica can be correlated with pitta and vata dosha vitiation. Inflammation is caused by pitta dosha and the pain is associated with vata dosha. And the management is based on the cause of the condition.

Causes and Risk Factors of Bursitis Trochanteric

Repetitive Stress Injury

Frequent climbing stairs, running, standing for longer duration, cycling can lead to Bursitis trochanterica.

Hip Injury

Traumatic bursitis can happen due to sudden fall onto the hip point.

Spinal Diseases

Diseases like lumbar arthritis and scoliosis can cause Bursitis trochanterica.

Unequal size and length of the leg

Walking uniquely to compensate for the leg length shortening can result in bursa irritation and ultimately Bursitis trochanterica.

Rheumatoid Arthritis and Gout

Rheumatoid Arthritis raises the risk of developing Bursitis trochanterica. Gout can result in the Bursitis trochanterica due to accumulation of urate crystals in the synovial joints causing irritation.

Prior Surgeries

Prosthetic hip and previous surgeries of the hip and around the hip are predisposing factors of Bursitis trochanterica.

Calcium deposits and Bone spurs

Bursae can get inflamed due to calcium deposits and bone spurs in the tendons.

Ayurvedic Overview

Ayurveda is a life science which describes how to lead a healthy life as well as mentions about the management of various health conditions. Ayurveda is based on the three doshas, seven dhatus and other vitals pillars that decides the health of a person and as well as diseased condition. Any imbalance of the dosha leads to a disease whereas balance of the dosha is regarded as health. Ayurveda also takes mental state as a part of standardisation of health as it mentions the happiness of the mind and soul are also important in determining the health of a person.

Hence, the pacification of the same is done through herbs such as haridra (Curcuma longa), nirgundi (Vitex negundo), ashwagandha (Withania somnifera), laksha (Laccifer lacca) etc. And there are other herbs that effectively help in bone health management such as asthi shrinkhala (Cissus quadrangularis). There are also some other mineral based formulations available that provide nutrients to the bone and help its diseased conditions.
