Afibrinogenemia: Introduction

Afibrinogenemia is a blood fibrinogen disorder because of which blood does not clot normally in affected individuals. The disorder is a genetically-inherited or congenital condition. It is characterized by uncontrolled/excessive bleeding because of the inability of blood clots to protect the body after an injury. Afibrinogenemia also called Congenital Afibrinogenemia is believed to affect nearly one in 1 million people across the world.

The occurrence of Afibrinogenemia is linked to the absence (or reduced levels) of the 'fibrinogen' blood protein because of mutations in one of the three fibrinogen genes alpha-chain (FGA), beta-chain (FGB) or gamma-chain (FGG). Fibrinogen is a blood coagulation factor the protein gets converted into 'fibrin' which is essential for the formulation of blood clots to stop bleeding after an injury. 

In people suffering from Afibrinogenemia, the level of fibrinogen protein in the blood is less than 0.1 g/L. The low fibrinogen levels result in bleeding abnormalities, which can be mild in some patients and severe in others. The first evident indication of the disorder is bleeding from the umbilical cord immediately after birth.

Afibrinogenemia patients are prone to hemorrhage or instances of severe bleeding, especially during the early years of life. The disorder generally affects the skin, oral cavity, nose, joints, muscles, liver, gastrointestinal tract, genitourinary tract, and central nervous system. Moreover, in some extremely rare cases, Afibrinogenemia can also cause intracranial hemorrhage (bleeding in the skull) which can be fatal.

Afibrinogenemia: Symptoms

The first symptom of Afibrinogenemia is hemorrhage from the umbilical cord at birth. Later in life, the bleeding abnormalities indicative of the disorder involve the following symptoms:

  • Excessive or uncontrollable bleeding after injury
  • Bruising easily even from a minor injury
  • Nasal bleeding which is difficult to stop
