Phimosis (literally meaning muzzle) is a condition where you find very tight foreskin, which makes it difficult to pull back the glans penis. This condition makes the penis non - retractable. It is one of the commonest conditions among teenagers and adult males.Phimosis can also occur physiologically in young children. But it may get resolved by the age of seven or may by age of 16.

Phimosis stays calm usually but triggers the pain when the penis gets erected. The condition is typically understood from the scarring of foreskin opening. Phimosis can be caused by either mechanical condition or pathological condition. At some instances it becomes very difficult to diagnose between physiological or pathological Phimosis, even if an infant shows discomfort in urination or have obvious ballooning of the foreskin. Mechanical factors which aids for the Phimosis development includes

  • Foreskin tip is too narrow to pass over the glans penis - normal among children and adolescents
  • Inner surface of the foreskin is fused with glans penis - normal among children and adolescent, but abnormal in adults
  • Frenulum breve - frenulum is too short to allow complete retraction of the foreskin

But for pathological Phimosis, the causes are varied and very rare to happen. This includes

  • Balanitis - inflammation of the glans penis
  • Lichen sclerosis et atrophicus - the most common and important cause for Phimosis. A cicatrix (a whitish ring of indurated tissue) formation near the tip of prepuce preventing the retraction of the foreskin leads to Phimosis. The exact cause for lichen sclerosis still remains unknown.
  • Balanoposthitis - inflammation of glans penis along with the foreskin
  • Repeated catheterization
  • Forcible skin retraction
  • Untreated diabetes - can lead to foreskin infection, due to the increased presence of glucose in urine

The severity of Phimosis can be varied in individuals. It can be either relative Phimosis or full Phimosis. In relative Phimosis, foreskin can be partially retracted and it is completely non - retractable in later one. Penis can be even become flaccid in full Phimosis.

Every male is borne with a sealed foreskin and glans penis. It remains non - retractable throughout the early childhood. Some may have to even wait up to 18 years of age, for their foreskin to be fully retracted.A comparable condition of the same for women can be seen and is termed as clitoral Phimosis, where the clitoral hood fails to retract. This limits the exposure of glans clitoridis.

Scale of Phimosis Based on the Severity of the Condition

  • Score 1 - full retraction of foreskin is possible but behind the glans it is tight
  • Score 2 - partial exposure of glans, prepuce limiting factor
  • Score 3 - partial retraction, meatus just visible
  • Score 4 - slight retraction, neither meatus nor glans can be exposed (some distance between tip and glans)
  • Score 5 - completely non - retractable foreskin

Physiological Phimosis, which is common doesn't require any treatment intervention, as it can be resolved by itself. During the course of puberty, it gradually becomes retractable. Circumcision is often regarded as the last resort. But proper medication and care is inevitable, if it is not causing any acute or severe problems.

There is a belief that childhood circumcision often prevents Phimosis. But it is still under question.

Herbal Remedies for the Management of Phimosis

The vitiation of the tridosha - vata, pitta kapha - accounts for the development of Phimosis. The predominance of dosha can be cited depending on the causative factor variant. 3. Boswellia - Curcumin Capsules

Combination of two drugs shallaki (boswellia serrata) and turmeric (curcuma longa). They are potent anti inflammatory drugs. They help to boost the immune system and also to modulate. They are mainly indicated to enhance the blood circulation in the body. Curcumin is a potent anti oxidant. Curcumin is very popular for its antibacterial action. It helps to relieve the pain and inflammation.

Dosage :- 1-2 capsule twice daily after meals with lukewarm water

4. Neem Capsules

If Phimosis left untreated, it may trigger much for complicated condition called as Paraphimosis, where the glans is heavily swelled and painful. It can also lead to urinary retention and carcinoma of the penis.
