The nose has tissues where the susceptibility genes required for infection are expressed at high levels. High levels of ACE-2 receptors on host cells, which may be modulated by environmental and host factors. In the nasal passages, goblet secretary cells, which produce mucus, express RNAs for both of the patients that SARS-CoV-2 attaches to infect cells.
In the lungs, they found the RNAs for these patients mainly in cells called type II pneumocytes. These cells line the alveoli or air sacs of the lungs and are responsible for keeping them open. Covid 19 as the SARS-CoV-2 spike protein receptor, ACE-2, is highly expressed on the nasal mucosa and type II airway pneumocytes.
Virus shedding has the highest probability from the nose and therefore reflects a major source for transmission of disease.
