Abstract

Hemolytic uremic syndrome is a clinical syndrome which is typically characterized by progressive renal failure. This syndrome is termed as micro angiopathy (disease of blood vessels) which is associated with hemolytic anemia (a type of anemia in which red blood cell destruction is more than its production) and thrombocytopenia (refers to low platelet count). In Ayurveda, HUS can be considered as a condition of pitta imbalance. The cause for this syndrome is some strains of Escherichia coli (E. coli) bacteria. The first symptom that usually appears is several days of diarrhea which is frequently but not always bloody.

Introduction

HUS is most common in children. It is a very serious disorder which is initiated by small blood clot formation. This clot formation can be observed throughout the body. These clots start blocking blood vessels supplying vital organs like kidneys, heart and brain. In hemolytic uremic syndrome hemolytic means breakdown of red blood cells and uremic means accumulation of urea in blood because of the damage done to kidneys. The damage to kidneys is usually due to lack of blood supply. The thrombocytopenia in this case is called thrombotic thrombocytopenic purpura (TTP) as too much unnecessary thrombi formation leads to abrupt lack of platelets in the body.

Causes

The basic cause is infection caused by E.coli.  Further this infection leads to hemolysis i.e red blood cell destruction, thrombocytopenia (low platelet) and mild to severe injury to the kidney.

Symptoms

The symptoms in HUS vary with respect to the system affected. Usually symptoms start from diarrhea with increased frequency and urgency of defecation. Most of the time diarrhea starts with passage of blood. Abdominal pain or cramps along with increased frequency of defecation is also noted. Gradually due to loss of excessive fluids from the body skin grows pale and the patient suffers from dehydration. Fatigue accompanied with fever is also observed. Slowly patients become irritated and start developing unexplained bruises on different parts of the body. The frequency of micturition is decreased and fluid starts accumulating in the abdominal cavity. Blood in urine (hematuria) may also be noticed. Swollen faces and limbs are also noticed. At last patients develop central nervous system related symptoms like confusion and seizures (rarely)

Diagnosis

Generally diagnosis is done on the basis of a blood test report showing low platelet count and low red blood cell count.Kidney function test may show temporarily increased levels of creatinine. Urine test may detect proteins and blood in urine. Stool test can also be done for the traces of bacteria and blood. In some cases occult stool tests (to detect blood traces which are not visible from naked eyes) are also done.

Treatment of Hemolytic uremic syndrome (HUS)

In modern science the Treatment for Hemolytic uremic syndrome (HUS) is hospitalization. After being hospitalized patient may undergoes following:

  • Fluid replacement: The fluid replacement is done to replenish electrolytes that are essential for the working of body. This also increases blood flow to the kidneys. The administration is through an intravenous route.
  • Transfusion: Red blood cells and platelets are also transferred through intravenous routes.
  • Dialysis: If infection is chronic and kidney functioning is excessively hampered then patients may need temporary dialysis of kidney.

These all are symptomatic management and are having severe adverse effects which can be even life threatening.

Revive kidney pack

The revive kidney pack has four products that are used for the management of many kinds of kidney disorders. It includes mutrakrichantak churna, Rencure formula, varunadi vati and punarnava mandoor .

1. Mutrakrichantak churna

This classical formulation is a powder blend of many herbs like punarnav, gokshura, varuna and many more. Punarnava (Boerhavia diffusa) is known for its diuretic actions (removing excess fluid content from the body). Gokshura (Tribulus terristis) and varuna (Crataeva nurvala) are known to decrease serum urea and creatinine. They also help in reducing inflammation and prevent infections. This formulation is very well known for managing all type urine and kidney related disorders.

Dosage: Take one teaspoon of churna and add 400 ml of water. Boil the content till it is reduced to 40-50 ml. use this decoction daily twice after meals. The decoction should be made fresh.

2. Rencure formula

Rencure Formula is available in capsule form which are made up of standardized extracts of herbal plants like plash (Butea monosperma), kassni etc. All the ingredients in this formulation are best to provide support to the kidneys.

Dosage: Two capsules daily twice after meals.

3. Varunadi vati

Varunadi vati is a classical formulation which contains shuddha guggulu (Commiphora mukul), gokshur (Tribulus terrestris), varun (Crataeva nurvala), and punarnava (Boerhavia Diffusa). Gokshur and punarnava are known for their diuretic actions and varuna is having diuretic and homeostasis properties. Varunadi vatti also used as a non hormonal preparation for managing benign hypertrophy of prostate and avoiding residual urine in void.

Dosage: Two tablets daily twice after meals.

4. Punarnava Mandoor

The formulation contains punarnava, chitrakmool, vidanga and many more. It is known to reduce swelling in ankles and legs especially due to water retention. The prescription of this medication is contraindicated in pregnant and lactating women.

Dosage: Two tablets twice a day after meals.

5. Plato Plan Syrup

The syrup contains multiple herbs that are known to boost immunity of a person like Eranda karkati (Carica papaya), giloy (Tinospora cordifolia), Wheat grass (Triticum aestivum) etc. This syrup is also known for its blood purification effects.

Dosage: Two teaspoon twice daily after meals.

6. Kutaj ghan vati

The tablet uses a single herb i.e kutaja (Holarrhena antidysenterica). Kutaj is having deepan (appetizer) and pachan (digestive) effects. It is also having anti diarrheal properties.

Dosage:Two tablets twice a day after meals.

7. Reno Plan Syrup

It contains herbs like kalmegh (Andrographis paniculata), bhumi amla (Phyllanthus niruri), trin panchmool ghan and many more. This helps in enhancing kidney functioning and maintaining kidney health.

Dosage: One teaspoon twice daily after meals.
